The QIDP is responsible for developing and overseeing the implementation of the Individual Support Plan for approximately 24 indi-viduals served. This position works collaboratively across agency programs and with external service providers to promote the deliv-ery of quality services to people served. The QIDP develops and implements person-centered plans that facilitate the achievement of personal outcome measures. This position is directly responsible for the oversight of services and supports across all program loca-tions. The QIDP is expected to uphold the Mission & Values of the Association.
Must have a Bachelor Degree in social services and at least 1 year of direct experience working with adults with developmental disa-bilities. Excellent oral and written skills are essential. Must have Clear State Police History and a valid Illinois Driver’s License. Must be at least 21 years of age. Must be able to lift and transfer adults (75lbs.) Must pass a pre-employment drug screen
Additional Salary Information: Wages/Salary: Commensurate with experience. (Non-bargaining Unit position)
