Established community mental health center in Skokie, IL seeks Bachelor's level individuals to serve as Full-Time Residential and Adult Case Managers in our scattered site residential program. This is a 40-hour per week position.
As part of the Residential and Adult Case Management team, this position works with clients to maintain/gain their independence and reach a higher level of functioning. This is done by providing direct services including but not limited to: assessing client’s individual case management needs, developing a treatment plan, assisting clients obtain benefits, linking clients to providers (therapist, psychiatrist, dentist, PCP, etc.), developing a monthly budget, helping clients maintain their apartment, and providing client support in their community.
This position requires travel, including a valid driver’s license, and time spent in the community with clients. Salary offered is $28,000.

Minimum Bachelor’s degree in Human Services or Social Sciences related field
Must possess valid Driver’s License and Current Vehicle Insurance
Communicates well orally and in writing.
Models professional behavior for staff.
Must be able to lift 10 pounds.
Working knowledge of securing IDHS entitlements/benefits and community resources a plus.
