The Social Worker/Inpatient Trauma Intervention Specialist provides primary assessment and care to trauma patients and families regarding health, mental health, and spiritual needs.
Responsibilities of the Social Worker/Inpatient Trauma Intervention Specialist will include:
Conduct psychosocial/spiritual assessments and orientation with Trauma patients and families, including conversations about advance directives. Complete assessments regarding the impact of trauma, ethical issues, spiritual and emotional health, and safety risks (re-injury and/or retaliation). Offer professional spiritual, palliative care, therapeutic, risk reduction, and bereavement interventions as appropriate. Make referrals to in-house and community providers as appropriate and available.
Provide support services to families
Provide outreach, psychoeducation, and information regarding HHP-C programming for eligible patients on the Stroger campus.
Provide follow-up outreach, psychoeducation, and information regarding HHP-C programming for eligible patients who are discharged from both Comer and Stroger hospitals.
Coordinate patient follow-up with HHP-C staff.
Maintain documentation and reporting mechanisms for patient/family interventions and HHP-C outreach.
Master of Social Work or Master’s degree in a related field of study
Knowledge of the target communities and the target populations
Experience re: crisis intervention, violence screening, outreach, and family assessments
Clinical experience in Level I Trauma and critical care settings
Comfort with outreach activities in high traffic, high stress environments
Knowledge of relevant formal and informal organizations in the target communities
Competence in Microsoft Office and research databases
