We are now hiring for a full-time Case Manager to join our Get In Chicago Team!
The Case Manager provides services appropriate to client population to maximize client potential and promote their optimal functioning in the community.
Essential Job Functions:
Case Manager will be responsible for some recruitment, assessment, case management and retention of youth that are in risk and at risk of justice involvement.
Must be familiar with working with youth involved in the juvenile justice system as well as those with truancy issues.
Will be responsible for home visits, school visits, consultation with parents/probation officers.
Must be familiar with creating Individual Service strategy plans, making referrals, conducting registration/orientation.
Bachelor's Degree in Human Services, Social Work, Psychology or Sociology required.
Familiarity with creating Individual Service strategy plans, making referrals, conducting registration/orientation.
Minimum of 3-5 years of experience working with at risk youth in the community.
Experience working with youth ages 13-18 years of age required
Experience working in low-socioeconomic communities and with families required.
Must be Bilingual - English/Spanish
Must have a vehicle, driver's license and insurance to conduct home and community visits.
