Education Graduate of accredited advanced practice program with certification as Family Nurse Practitioner, Illinois advanced practice license and DEA Registration required.
Knowledge/Skills RN/NP skills necessary to perform physical exams. Bilingual in Spanish/English preferred.
Type/Years Experience Greater than 1 year primary care experience for men, women and children. Home care or home visit experience a plus.
Job Summary: Under collaborative agreement with a VNA Health Center physician, provides comprehensive medical evaluation, education and management for clients throughout the life cycle that present for routine preventive, acute and chronicdisease health services at the VNA Health Center clinic sites (off site clinics and residential visits).
