Care Coordination Nurse – Call Center Based (Manage Task Queue and Phone Triage)
Understands and accepts responsibility for all professional care duties performed. Follows accepted standards of nursing practice. Provides phone triage and task queue management for all patients served by the department, as assigned.
Answers and returns patient related calls. Communicates and coordinates accordingly with clinical and provider staff.
Renews and orders patient prescriptions, while ensuring accurate order information in patient’s medical record.
Assesses patient’s needs, develops and coordinates an individualized plan of care, including involving the client/family in the development of the patient’s goals and collaboration with other health care professionals as necessary.
Initiates the nursing interventions identified in the plan of care, per applicable Standing Orders and Protocols, evaluates effectiveness and outcomes of care, revises as needed.
Notifies patient and initiates follow up per applicable protocols for lab and/or mammogram results.
Handles complex case management of patients and ensures care continuity through all sites of health care services - clinic, Emergency room, inpatient and home based services.
Teaches self-care, mental and physical well-being and promotes independence and patient rights.
Reinforces the importance of preventative care and appropriate use of emergency versus primary care services.
Evaluates the effectiveness and outcomes of care. Determines if patient goals are being met and makes changes accordingly. Communicates these changes to the patient care team.
Completes timely, current and accurate documentation for all patient care activities. Follows organization documentation guidelines including electronic and paper processes.
Completes training, as requested.
Assumes responsibility for own professional development. Participates in continuing education programs and in-service programs. Completes continuing education as indicated by organization accreditation requirements.
Attends organization and team meetings as required.
Familiarizes and adheres to established VNA Health Care policies and procedures. Makes recommendations for new policies and procedures and assists with development, interpretation and implementation based on area of expertise in nursing.
Follows infection control and infection prevention per protocol.
Demonstrates sound judgment in decision-making.
Recognizes own behavior patterns, stresses and attitudes which impact client care. Explores methods of dealing appropriately with emotional strain.
Meets organization productivity requirements.
Fulfills holiday, on-call and weekend rotation requirements.
Participates in the organization's student education program.

Follows established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer (Director of Quality, Education and Risk Management) immediately. Failure to comply with HIPAA policies and procedures will result in disciplinary action, up to and including termination of employment.
Other duties as assigned.
BSN preferred. Current Registered Nurse License in the State of Illinois under the Illinois Nursing Act.
Experience: Two or more years of generalized nursing experience preferred, preferably with Community Health experience.
Knowledge and Ability: Comprehensive knowledge and skill in current nursing practice; concept of multidisciplinary team approach to nursing care; knowledge of professional limitation and scope of practice within the Illinois Nursing Act.
High degree of sensitivity to the rehabilitation needs of patients and their families and the ability to establish constructive working relationships with them.
Maintains current CPR certification.
Owns and operates a car.
Maintains current driver's license and car insurance.
Computer skills required.

Essential Functions/Physical Requirements:
Ability to perform tasks involving physical activity which includes heavy lifting and extensive bending and standing.
Ability to communicate effectively verbally and in writing.
Tactile, visual, auditory and olfactory senses intact.
Ability to climb stairs and ambulate in and out of patient's homes in all weather.
Ability to drive a car.
Manual dexterity sufficient to perform nursing procedures.
Ability to deal effectively with high levels of stress.
Ability to wear a particulate respirator during patient care as necessary.
