Case Management/Social Services, Program Management
Under the guidance of the executive director, the director of social services serves as the chief program officer responsible for oversight of all programs and services of the agency, including state and federal contracts, private grants and United Way awards.
This senior staff position combines clinical expertise, program management and overall administration of the social service department. Flexibility, commitment and drive are critical to success in managing the varied and competing demands of the position. Key elements and responsibilities include:
Plan and make recommendations to the E.D. on policy standards and procedures with regard to all social service activities of the agency.
Overall responsibility for staffing decisions across all program areas with support from supervisory staff.
Responsibility for the development and successful implementation of grant initiatives in keeping with the mission and strategic planning of the agency.
Oversight of Title III of the Older Americans Act services, Care Coordination and Adult Protective Services contracts and private grant initiatives.
Sets the tone and direction for all work of the social service department including comprehensive assessments in the home of older persons and ongoing care coordination.
Serves as a member of the senior leadership team.
Master's level social worker with supervisory experience required.
Experience with older adults and families preferred; knowledge of Illinois Department on Aging contracted services and Older Americans Act programs helpful.
Experience in managing programs, teams and multiple demands a must.
Aging Care Connections is a community based organization in suburban Cook County. ACC offers a wide range of programs and services to older adults and their family members. Programs and services are funded through State of Illinois contracts, Federal grants and private grants.
