Collaborates with the Behavioral Health Outpatient Treatment Teams to provide community support and psycho-social rehabilitation team services to individuals assigned to BHOP in order to decrease mental health dysfunction and increase community functioning,
Assists clients to achieve recovery goals.
Assists with assessments, supports the development of treatment plans and delivers evidenced based care in collaboration with the client, other BHOP staff, other providers, and other parties the client identifies as important to their treatment.
Completes all required documentation in accordance with the standards of Rule 132, payers, other applicable licensing and accreditation bodies.
