Summary: This position provides nursing services to individuals served. This position is accountable and responsible for planning, directing, and evaluating all activities of the Nursing Department, including implementation of the department's goals and standards (Rule 116), standards for nursing practice, and the management and development of nursing personnel.
Essential Functions * Provide 24-hour nursing (on-call) support for Residential Programs including 24-hour CILA, ICILA, and Foster Care. Respond to medical/behavioral emergencies or request for consultation in Residential and Day Program services. The Director of Nursing may need to evaluate consumer during evening and weekend hours at Residential site. * Monitor the day-to-day functions of nursing and health care services in all Residential Programs, to ensure each consumer receives consistent and appropriate care according to Rule 116, the Department of Human Services, and other licensing and regulatory agencies. * Development of appropriate policies and procedures related to consumer health care in all Residential Programs. * Implement an effective, ongoing program to measure, assess, and improve the quality of nursing care, treatment, and services delivered to consumers. * Participate in evaluation of current services and programs, proposed changes and expansion of services and programs. Assist management in selecting quality indicators. * Provide leadership, support, and supervision to staff that directly reports to the Director of Nursing. * Interview and hire potential new hires and makes recommendations for hire to the Director of Residential Services. * Be an active member of the Intervention Review Committee and any other necessary agency committees. * Collaborate with pharmacy as needed to ensure prescription orders are filled correctly, communicate any medication changes as needed, and ensure monthly medication check-in follows all physicians' orders. * Participate in monthly psychiatric reviews with provision of data related to psychiatric issues and seizure tracking. Performance of a Tardive Dyskinesia Screen when indicated.
Education * BA required; BSN preferred.
Experience * Three years nursing experience; preferred in the field of nursing with persons who are developmentally disabled and residential services. * Three years supervisory experience.
License/Certification * Illinois licensed RN, in good standing. * Approved DHS Nurse-Trainer. * Tardive Dyskinesia Screener Certificate.
Language * Excellent oral and written command of the English language. * Second language a plus, but not required.
Additional Qualifications * Proficiency in the use of computers and good typing skills. Proficiency with Word, Excel, and Outlook software. * Valid driver's license, with a good driving record. * Sertoma Centre provides service to "at risk populations" funded primarily by the State of Illinois. Industry Knowledge * Knowledge of medical issues common among individuals with developmental disabilities and aging populations. * Medication administration management and coordination of pharmaceutical delivery services. * Maintain an awareness of new developments in the field in order to promote and develop state of the art department programs and services for the agency.
Knowledge of Rules and Regulations * Knowledge of HIPAA requirements. * Meet requirements of applicable licensing, funding, accreditation and other state/federal regulatory agencies; including safety requirements and agency policies and procedures, through utilization of systems to ensure compliance with all standards. * Knowledge and understanding of DHS Administrative Rules 115 and 116.
Additional Job Responsibilities * The Director of Nursing is responsible for ensuring that all areas of Residential medical services are in accordance and compliance with the DHS, CARF, and Rule 116 (24-hour CILA). * Function as client advocate ensuring the protection of individuals' rights and communicating requested or needed services to appropriate personnel. * Provide leadership to staff to ensure the provision of quality services that are responsive to the expressed desires and interests of individuals served, support the concept of self-advocacy, and ensure the protection of rights. * Chair and conduct regularly scheduled nursing meetings. * Act as a resource to employees, guardians, and consumers for medical information and recommendations. * Attend and participate in IDT meetings, when possible, to provide input relative to the care plan objectives, as related to the consumer's health status. * Represent the values and mission of Sertoma Centre, Inc. to other professionals, the general public, individuals served, and agency employees. * Provide leadership to staff to ensure the provision of quality services that are responsive to the expressed desires and interests of individuals served, support the concept of self-advocacy, and ensure the protection of rights.
Customer Service * Work collaboratively with other agency staff and external customers to enhance communication, maintain positive work relationships, identify the needs of others, provide timely response, and promote teamwork for the common purpose of internal and external customer service. * Ensure the dignity of individuals served by treating them with respect and understanding at all times. * Provide open and authentic feedback to staff on a regular basis related to performance utilizing active listening skills and allowing constructive feedback. * Communicate with consulting physicians and other health care disciplines to ensure that the health needs of all consumers are being met. * Communicate with other programs within the agency to ensure consistency in the monitoring of health issues and provision of appropriate interventions.
Recordkeeping * Complete all required agency management reports, billing, personnel documents, and others as required in a timely and accurate manner. * Review and/or transcribe physician orders. * Oversight and review of all nursing documentation related to nursing services including nursing notes, quarterly and annual reviews, consents, etc. * Monitor the arrangement and scheduling for consumer medical appointments and obtain required consent from parent/guardian if needed. * Monitor and review all participant lab work, as ordered by the physician, and contact physician as needed in regards to concerns or other needed follow up. * Review and approve medical bills for consumers. The Director of Nursing will contact physician or office in regards to any needed adjustments of bills or insurance information.
Safety * Responsible for maintaining a safe workplace by following safety procedures, implementing good housekeeping practices and proper materials handling, enforcing the use of personal protective equipment when necessary, and reporting all unsafe acts and conditions. * Director of Nursing will provide nursing services and support to Day Program as needed. * Actively practice concepts established in safety training, policies and procedures, to promote a safe, clean working environment. * Report and assist in investigations of work related injuries and illness in a timely manner and in compliance with established Agency policy and procedure. * Actively assure the safety of consumers, employees, and visitors by identifying hazardous conditions and/or at risk behaviors and intervening to correct potential risks.
Training * Oversee and provide all medication administration training for employees in compliance with Rule 116. * Develop and provide needs specific training to employees related to medical needs of the consumers. * Develop and implement training programs that may be utilized for training newly hired nursing staff or other agency supporting staff in regards to safety and health management. * Attend all assigned training sessions, notifying supervisor of needed schedule changes to accommodate training date/times. * Maintain current status on all certifications. Monitor training history and schedule retraining for completion prior to certification expiration dates.
Policy, Procedures and Regulations * Maintain knowledge and adherence to all department and agency policies and procedures. Must stay current on policies contained in the Employee Handbook. * Assist with the development of agency policies and procedures, develop departmental procedures as needed to ensure efficient operations and maintain current departmental procedures manuals.
