The Transition of Care Cooordinator is responsible for making referrals for each discipline throughout NNHSC. Participates in the Quality Improvement audit process. Collaborates with the nursing and provider staff regarding the referral process. Communicate frequently with outside agencies. Exhibits professional attitude toward patients, visitors, providors and co-workers.
Will follow role assignemnts that are in alignment with the Patient Centered Medical Home Standards, and participate in all Patient Centered Medical Home efforts.
Makes internal and external referrals via fax, telephone, and Internet.
Document all referrals made in Referral Log Book
Reports weekly and monthly statistic of referrals made and where they were made for each site weekly to the Director of Nursing
Assists in establishing policies and procedures related to the referral process
Keeps accurate records
Ensures that referrals are processed within 24-48 hours
Demonstrates a genuine concern and passion for continuous quality improvement
Demonstrate and apply knowledge of medical terminology, high proficiency of general medical office procedures including HIPAA regulations
High school diploma or equivalent. Current State of Illinois LPN license and CPR.
Occasional Saturday or evening hours, and/or may be required to work at a different site
