POSITION TITLE: Fatherhood In Action Facilitator
LOCATION/DEPT: Family and Community Services
REPORTS TO (TITLE): Fatherhood Program Manager

STATUS TYPE: Regular Full-Time
To support the full mission of The Salvation Army Family and Community Services: “Empowering People to Live Healthy, Safe, and Contributing Lives.”
Will facilitate the 40 hours of the parenting and relationship components for the Fatherhood In Action (FIA) program, as well as provide on-going case management services for the cohort participants throughout their enrollment in the program. The Facilitator will work closely with the financial education and employment readiness services providers to create a continuity of care for each participant. Facilitators will be available to participants to provide after care services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provides, within agency policies, programs, and procedures, direct casework services to individuals enrolled in the FIA program.
Facilitate teaching two of the components of FIA program. Facilitators will use the On My Shoulders curriculum to address healthy fatherhood and parenting and will use the Within My Reach curriculum to address healthy partner relationships.
Work collaboratively with the providers of the financial literacy and employment readiness component of the FIA program.
Work closely with the Fatherhood Program Manager and Co-facilitator in assessment of participants, monitoring of progress, and evaluation of outcomes.
Maintain, in a timely manner, required documentation of participant's involvement including but not limited to assessments, demographic data, case notes, closing recordings, and outcomes data collection.
Obtain appropriate releases of information.
In consultation with the Fatherhood Program Manager, initiate contact with the Illinois Department of Children and Family Services as a mandated reporter of suspected child abuse or neglect.
In consultation with the Fatherhood Program Manager, initiate contact with the Elder Abuse Hotline as a mandated reporter of suspected elder abuse.
Participate in weekly supervision and case consultation with Fatherhood Program Manager. Seek consultation from the Program manager as needs arise.
Maintain awareness of applicable liability issues and other applicable legal issues, and potential effect on clients and/or the agency.
Abide by the ethical and culturally competent standards as outlined by the National Association of Social Workers and The Salvation Army.
All other duties as assigned.
Awareness of agency policies and procedures.
Attend agency general staff meetings and professional trainings as applicable.
Perform special administrative assignments or duties as may be delegated.
Understands, and can effectively work under the Mission of The Salvation Army and agrees with the core values of Family and Community Services.
Attend meetings with community partners, as needed.
Participate in community events as part of public relations and marketing of the Fatherhood In Action program.
The minimum educational requirement for the Fatherhood In Action Facilitator position is a Bachelor's Degree in social work, psychology, sociology, or related field with a case management experience, a Master's Degree is preferred.

Driver's license, MVR clearance and a working, appropriately insured automobile preferred.
PHYSICAL DEMANDS/WORK ENVIRONMENT
The position is required to do light physical work.
In order to successfully perform the essential functions of this position, the facilitator is required to use standard office equipment.
The work environment for the position includes office environment and classroom setting with a low noise level.
The position requires the ability to go off site and working some evenings.
