• Responsible for case management, advocacy, and individual counseling to both youth and adult clients in the Inspiring Hope Program • Conducts intakes and assessments and timely individualized service plans for each participant which includes specific measurable short and long term goals and objectives. Monitors client’s progress through routing office visits with all clients. • Provides relocation services to clients as needed • Works independently and consistently exercises discretion and sound judgment in providing supportive and crisis counseling to clients • Maintains up-to-date and accurate client files and databases, as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies. • Knowledgeable about community resources and links clients to appropriate resources. Advocates on behalf of clients in order to ensure access to services for which the client is eligible. • Coordinates client services with the program’s therapists in order to help ensure the client is able to meet short and long term goals related to individual therapy. • Assists with Program’s Performance and Quality Improvement activities. • Solicits, logs, sorts, and transports and places in storage in-kind donations for the program. Delivers needed basic household items and other resources to client’s home when necessary. • Participates in community meetings and represents the agency regarding services provided as well as providing accurate and timely reporting to the Clinical Manager of the meeting discussions.
Preferred Requirements: MSW. Three years of experience. LCSW/LCPC
Minimum Requirements: Master’s degree in related field. Two years of experience.
