Under the direction of the Manager of Patient Centered Care, you will be responsible for ensuring the teams’ vision and PCMH standards are implemented; and that team roles and expectations are known and clear. As a qualified team leader, you will motivate, provide effective guidance, communicate with the multi-disciplinary team members on a daily basis, and be the contact person for all team members. You will act proactively to ensure smooth team operations and effective collaboration--creating an inspiring team environment with an open communication culture. As the Clinic Team Leader, you will be responsible for monitoring the PCMH clinic sessions, the patients empaneled to the sessions and facilitate effective communications with the multi-disciplinary team members.
To develop the clinic teams’ goals based on PCMH requirements, expectations and effective communication
Facilitate effective communication between patients, team members, disciplines and administrators
Monitor team and clinic performance using data metrics and participate in continuous quality improvement efforts
Encourage staff development and team building efforts.
Oversee the teams’ coordination of internal and external referrals and monitor processes
Adhere to Department’s Documentation Policy of all Patient Encounters and Forms.
Present PCMH outcomes internally and externally as needed
To coordinate with the clinic team members quarterly PCMH meetings.
Participate and actively contribute in the Clinical Care Coordination Committee
Maintain database and submits reports based on population health metrics, HRSA/HAB performance standards and PCHM measures
Other administrative functions such as assistance with the development of abstracts
