Under the direction of the CORE Center Prevention & Education Coordinator and the HIV Testing Coordinator, assess individual and group health education needs in the Screening Clinic and in community settings; maintains up to date knowledge base of HIV transmission, treatment and disease, performs pre and post HIV counseling and testing and other related HIV issues. Additionally, conducts and evaluates group and individual health education sessions as related to HIV and STI prevention and risk reduction including condom demonstrations. Identify regular Screening Clinic patients/visitors, facilitates video intervention viewing to identified high risk patients and request feedback after the video was viewed. Provides linkage to care for patients who test HIV positive and makes assessments and referrals to the PrEP Clinic for high risk HIV negative patients. Develops and maintain project database including data date entry in EvaluationWeb. Analyze data and submits reports. Develops, implement and provides patient education in related topics (i.e. STDs, hepatitis C, medication adherence, etc.). Participate in all administrative meetings and staff development meetings, representing health education and prevention issues, activities, goals and outcomes.
Provides and monitors Safe in the City Program in the Screening Clinic.
Conducts project surveys.
Collects and enters data related to project.
Provides test results, post-test counseling and assess client risk behaviors.
Refers clients to follow-up medical care and other prevention programs as needed and schedule future appointments as necessary.
Educate, assess and refers high risk patients to PrEP services.
Provides patient, provider and community education on HIV and related topics.
Assess client’s interest in peer education and refer for training as appropriate.
Generates monthly report of Health Education activities.
Generates reports to funders and administration.
Bachelor’s degree in a health related field, education, psychology, social science or a minimum of five years of counseling required. One to three years’ experience as an HIV related counselor or educator required. Bilingual desirable (Spanish-English). Ability to work with a diverse population. Ability to work within a multicultural and multidisciplinary team. Strong oral and written communications skills. Strong computer skills, Ability to meet deadlines. Ability to be a team player.
