Under the general supervision of the Co-Investigator, provides community referrals, counseling and health education to the pregnant females at Cook County jail. Referrals are made for in-house prenatal care, nutrition, smoking cessation, substance abuse treatment, psychosocial, STD and other issues. Must be comfortable working in correctional environment and with detainees.
Job responsibilities include, but not limited to:
Conducts client intake, periodic assessment and care planning for clients
Attends monthly and quarterly meetings
Educate detainees on HIV, STD, Harm Reduction , Prenatal Care
Input into the State of Illinois V-Stone system applicable data
Demonstrates ability to be self –directed
Demonstrates effective verbal and written communication skills
Ongoing care management assessment of service’s needs, developing linkages with medical partner organizations and referring detainees to appropriate community services

Qualifications/Basic Job Requirements:
A minimum of a bachelor’s degree in social services, behavioral, community health , nursing or other related field
Must be or become V-Stone trained or skilled
Excellent oral, written and computer skilled
Must be highly organized and able to work pressure,dynamic,creative and motivated self-starter
Must have a valid license to be able to drive to mandatory community meeting and training
