Alivio Medical Center's is seeking a full-time Program Coordinator for it's Age Options program, targeting culturally and linguistically isolated persons (TCLIP). The TCLIP Program Coordinator, a grant-funded position, is responsible for all aspects of the TCLIP program, including working with key community stakeholders, establishing partnerships with agency currently charged with meeting the needs of older adults in their service areas. With the support of the Get Covered Illinois and Senior Programs Manager, the TCLIP Program Coordinator is responsible for all aspects of the TCLIP program, including activity reporting; also assists Alivio’s Development and Finance Departments on fiscal reporting.
Reaches out to key community stakeholders to get commitments to identify current services, gaps in service and develop process for improving access and delivery of services for seniors and their families.
With community stakeholders, develops a plan to address service improvement, delivery and access.
Works with community partners to identify, train and deploy volunteers and other advocates to provide outreach, educational and direct service to area older adults.
Provides input to the Senior Programs Manager and the Alivio’s Development Department to develop programs and submit proposals to various entities, and helps identify funding sources.
Participates in the committee and coalition meetings as assigned.
Develops and maintains formal relationships with local, state and federal organizations to improve access to programs.
Other related duties as assigned.
Bachelor's degree and/or related experience in senior programming
Senior Health Insurance Program (SHIP) certification and ACA-GCI Certified Application Counselor, In-Person Counselor and/or Navigator License preferred.
3 years experience working on community organizing and/or outreach programs with Latino and/or Senior population. Has some experience in program operations.
Possesses excellent written and oral communication skills.
