The primary purpose of the Resource Coordinator is to provide service facilitation to individuals and families in the Home Based Services program. The Resource Coordinator is responsible for developing and overseeing the implementation of the Individual Support Plans of individuals served. This position works collaboratively across agency programs and with external service providers to promote the delivery of quality services to people served. The Resource Coordinator supports individuals and families to connect with community resources that promote the facilitation of personal outcome measures and develops and implements person-centered plans. The Resource Coordinator is expected to uphold the Mission & Values of the Association.
Must have a Bachelor Degree in social services and at least 1 year of direct experience working with adults with developmental disabilities. At least 1 year experience as a QIDP. Excellent oral and written skills are essential. Must have Clear State Police History and a clear and valid Illinois Driver’s License for at least 1 year. Must be at least 21 years of age. Must be able to lift and transfer adults (75lbs.)
