As a team member who directly serves seniors, you will work with highly-skilled, dedicated colleagues.
We are currently looking for a Case Manager to join our team in our Northfield office.
This role provides general case management and Community Care Program (CCP) services to clients residing in our service districts.
Meets face to face with clients in the office and in the home (residences in elevator buildings and non-elevator residences).
May conduct a Choices for Care assessment for clients in the hospital or nursing facility.
Implements and monitors care plans.
Arranges with vendors to provide service to clients.
Provides clients and family members with emotional support when indicated.
Provides information and referral to the elderly and their family members.
Makes referrals to community agencies as appropriate. Refers to Adult Protective Services (APS) agency when an abusive situation is present (Niles only).
Meets with program supervisor for regularly scheduled supervision.
Maintains CCC and CCP certifications.
Completes statistical data and required paperwork in a timely manner.
Attends scheduled staff meetings.
Completes a minimum of 18 hours of continuing education each year.
Responds to medical emergencies and performs CPR/AED and First Aid if certified.
Performs other duties as may be assigned.
Bachelor’s degree in social services or related field required
Master’s degree in social work, counseling, or related field preferred
One year experience in social services provision setting to older adults required
Knowledge of community supportive services, CCP (Community Care Program) experience preferred
Sensitivity to cultural diversity
Knowledge of basic computer programs
Current driver’s license and access to a car required
Free of communicable diseases as determined by an annual TB test
Clear criminal background check
Capable of making visual assessments of the home environment
Ability to climb stairs
Ability to lift and carry 25 lbs.
Current CPR/AED and First Aid Certification preferred

Since our founding in 1956, North Shore Senior Center has partnered with older adults to redefine the process of aging. We imagined a social community for seniors -- one defined by friendships, volunteerism, clubs and ne...w experiences. We didn't know what it would look like, but we knew that creativity, enthusiasm and knowledge would be its foundation.

Today, thanks to the collective efforts of our members, staff, volunteers and friends, the North Shore Senior Center community we imagined is a reality. Whether you visit us to take a class, enjoy a concert, or exercise in our Fitness Center, you can experience its benefits.
We also imagined that our community could extend beyond the walls of our brick-and-mortar buildings. It was important that older adults who were isolated due to physical, emotional or financial issues could participate as well. It was equally important that North Shore Senior Center could help those who needed assistance with Medicare, income taxes, caregiving concerns and issues relating to Alzheimer's disease and memory loss.
To ensure that all the pieces we imagined fit together perfectly, we strengthened our community with the passion of volunteers and the commitment of our partners in local businesses and government.
