Summary of Position: The Case Manager – Shelter and Community Programs, provides direct service to residents of the Hilda’s Place shelter, past shelter residents receiving aftercare services and individuals accessing services at the drop-in center.
Conduct comprehensive intake assessments and screen potential residents for eligibility
Provide case management services to Hilda’s Place residents
Develop case plans and goals in collaboration with Hilda’s Place residents and monitor progress while in the program
Maintain participants’ files and case notes and prepare reports and other required documentation completely and in a timely manner
Enter client level data in HMIS
Effectively engage potential participants and explain the scope of services provided by Connections for the Homeless
Network with area service providers in order to effectively advocate for Hilda’s Place and Drop-in Center participants and provide appropriate resources and referrals when needed
Present during Drop-In Center hours of operation to provide case management services and support the milieu
Assist participants in securing stable housing upon program exit
Meet with former Hilda’s Place clients who request aftercare services and provide case management services in the field or on-site
Bachelor’s degree in social work or related human services field, or equivalent combination of education and experience
Strong communication skills with a demonstrated high level of empathy and respect for program participants
Computer competency and comfort with data entry
Experience working with individuals experiencing homelessness and/or mental illness preferred
