Access Living seeks a full-time employee to serve DON Assessment Coordinator. The position will be responsible for conducting Determination of Need Assessments, as requested by the State of Illinois.
Qualified candidates should have a majority of the following qualifications:
Coordinates and prepares monthly, quarterly and annual reports where necessary;
Conducts required consumer/project file and electronic data entry;
Maintains confidentiality through appropriate consumer file maintenance;
Creates and implements program evaluations;
Experience (personal or professional) working with people with disabilities (as peers);
Strong written and oral communication skills so to connect with a wide range of individuals and organizations;
An understanding of equal opportunity within a social justice framework;
Knowledge of disability civil rights laws (Americans with Disabilities Act, Rehabilitation Act: Section 504, and the Fair Housing Act As Amended), and
Other duties as assigned.
EDUCATION AND TRAINING:
Master's Degree with major course work in rehabilitation, counseling, guidance psychology, or a closely related field, plus one-year of professional experience, e.g. social worker with a Master’s in Social Work); a registered nurse, licensed in the State; or a Licensed Practical Nurse (LPN) or Vocational Nurse, acting within the scope of practice under State law.
Access Living prefers the staff have experience with independent living and/or social work practice and theory and the ability to find needed community services. Bonus qualification is the understanding of person centered planning and the long-time segregation of people with disabilities in nursing facilities.
Capacity to travel locally within the Cook County.
