The QDDP (Qualified Developmental Disability Professional) is responsible for completing assessments, individual treatment plans and documentation, as indicated, for a designated caseload of persons receiving services and supports. In addition, the QDDP/assessment coordinator is responsible for maintaining a clinical file for persons on their caseload as well as scheduling and facilitating plan review meetings twice per year and documenting services provided in accordance with various Illinois standards and regulations. The position also requires that the QDDP/assessment coordinator assist in preparation for anticipated as well as announced surveys/audits. Additional tasks may be assigned based upon the needs of the department.
The position is full time; M-F 3 days 8:30-5:00pm, 2 days 12:30-9:00pm.
At minimum, a Bachelor’s degree in a Social Service or Human Service field, is required. One year experience providing direct supports to those with an intellectual/developmental disability is required. Must demonstrate proficient communication, writing and organizational skills.
