The Family Case Manager (FCM) works with physician practices and outpatient teams to promote the effective utilization of services and coordination of care for patients and their families. The FCM contributes to the team’s effectiveness by reviewing plans of care, identifying barriers to effective and efficient utilization of resources, and appraising patients’ psychosocial, financial and clinical needs throughout the continuum of care; coordinating with all relevant TCA departments and programs; and functions as a liaison between patients, physician practices, the clinic, and the community and ensures that patients’ continuity of care needs are met.
Required Skills and Qualifications
To work with the families of TCA including those in Adult Medicine, OB/GYN and Pediatrics by bringing patient centered care to the forefront of our efforts at TCA.
Coordinates the development of health care through a comprehensive assessment using Hollister forms, HEADSS and Bright futures and establishes priorities for the patient as a member of the interdisciplinary team.
Makes home visits for high risk patients.
Assists in training new staff at various project sites

Education and Experience
Bachelor Degree in Social Work or Human Services.
At least 1 year experience working with adults, pediatric and prenatal clients strongly desired.
Current CPR certification. Organizational ability to plan and direct clinical and personnel activities.
Experience in case management of obstetrical, pediatric and adult patients highly desirable.
Conscious of age specific medical needs of all populations to be served.
