Under the direction of the CORE Center’s Director of Prevention & Education and the Community Testing Coordinator, s/he will assess individual and group health education needs on site and in the community; maintains up to date knowledge base of HIV transmission, treatment and disease, interpretation of HIV test results, other related HIV issues, delivers prevention messages to clients in HIV Primary Care clinics and makes referrals to PS programs as necessary. Will provide on-site community based HIV testing as needed. Flexible hours to include evenings and occasional weekends. Additionally, conducts and evaluates clients’ health education sessions as related to HIV prevention and risk reduction; provides pre and post HIV counseling and testing and follow-up with clients; performs, develops, and provides professional, community and peer educator HIV education training. Develops, implement and provides patient education in related topics (i.e. STDs, hepatitis C, medication adherence, etc.). Participates in all administrative and staff development meetings, representing health education and prevention issues, activities, goals and outcomes.
Performs pre and post HIV counseling and testing with adults, adolescents and children throughout various community sites and Cook County Bureau Campus.
Provide patient and community education on HIV and related topics.
Assists with development of peer educators
Delivers prevention messages to clients in HIV Primary Care clinics on an as-needed basis.
Enters data related to HIV testing and assists with reports
QUALIFICATIONS: Bachelor’s degree in a health related field, education, psychology, social science or a minimum of three years of counseling required. Bilingual required (Spanish-English). One to three years experience as an HIV related counselor or educator a plus. Ability to work with a diverse population within a multicultural and multidisciplinary team. Strong oral and written communications skills. Ability to meet deadlines and be a team player. Moderate knowledge of word processing and basic skills, spreadsheets, and database applications. A car and valid driver’s license required.
