Summary of Position: Provide general case management services to clients seeking homeless prevention and rapid rehousing services.
Responsible for processing all prevention and RRH (rapid rehousing) requests for financial assistance
Provide referrals to outside providers when participants do not qualify for Connections-administered funds, or when all Connections’ funding has been disbursed
Outreach to area schools, agencies and other stakeholders to inform people about the program and to identify potential participants
Handle intake and assessment calls and callbacks
Conduct interviews and assessments leading to program qualification
Assist clients in identifying and locating services that will help them reach their goals
Conduct follow-up client interviews to monitor progress on achieving goals laid out in their service plan
Provide case management as required to ensure participants are able to comply with funder requirements
Maintain relationships with other service providers and participate in planning and service coordination meetings and activities as needed
Represent agency at community service provider and other meetings as assigned
Experience working with people experiencing homelessness, domestic violence and/or clients impacted by trauma
Valid Illinois Driver’s License and must be insurable under agency policy
Flexible schedule—able to work some evenings and weekend
Able to work independently and with diverse populations and able to meet clients in their homes
Good speaking, writing, and interpersonal skills
Computer literacy in Microsoft Word and Excel and in client database usage
Bachelor’s degree preferred
Basic knowledge of HUD and state funding requirements preferred
Experience with State of Illinois or other prevention or rapid rehousing funding preferred
