Provides quality individual service coordination to clients with intellectual/developmental disabilities and their families who reside in McHenry County. Duties include developing a person centered individual plan for each person in compliance with DHS/Department of Developmental Disabilities rules and guidelines, monitoring service delivery in-line with the plan, maintaining client records in compliance with Medicaid waiver and funder requirements.
Ability to manage 60-70 person caseload, ability to work well within a team as well as independently.
Travel within McHenry County.
QIDP preferred. Minimum of Bachelor's degree in a Human Service field and one year of experience working with individuals with IDD.
