Erie Neighborhood House, a progressive social service agency serving the Chicago metropolitan area and dedicated to promoting a just and inclusive society, is seeking a full time Community Support Specialist to provide direct support and assistance to the Proyecto Cuídate Program in Erie’s Little Village location. The basic function of this position is to support a team of professional providers serving immigrants in the Little Village area of Chicago. Proyecto Cuídate provides individual, group and family services to support, inform and educate the community on topics that strengthen the individual and family unit. It includes:
Informative workshops and trainings in areas such as violence prevention, health & wellness, parenting skills, etc.,
Supportive counseling and case management.
School based and after school youth programming
Conduct psycho-social intake assessments, case management support, and crisis intervention and provide referrals to other supportive agencies as appropriate.
Co-facilitates or facilitates parenting classes, support groups and other program workshops at Erie’s sites, partner organizations or schools.
Conduct home visits as needed.
Creates and maintain client files in accordance with program documentation requirements.
Collects data, tracks participants’ attendance, and completes data entry on a timely manner.
Supports in planning community outreach and other recruitment activities through the development and maintenance of partnerships and collaborations in the community.
Represents Erie Neighborhood House to the public and actively participates in community meetings and events as required.
Attends monthly department, all staff, funder and other required meetings.
Performs other duties as required.
