The Case Coordinator is responsible for providing intensive outreach to the Family Works population and facilitates their engagement in services.
The Case Coordinator is responsible for assisting clients with completion of identified goals and assisting clients with resolution of identified barriers to goals. The Case Coordinator will complete intake assessments and make internal and external referrals to address client's service needs. The Case Coordinator also provides initial outreach on lease compliance referrals and is responsible for engaging youth in Youth Services.
Working and Reporting Relationships:
Reports to the Program Manager and works in collaboration with E&ES staff. Will also maintain positive relationships with CHA staff and partner agencies.
Functions and Duties:
Caseload includes CHA and HCV
Prepares files for CHA
Outreaches clients via various outreach methods (In-Person, Telephone, Email/Mail correspondence, Social Media, etc.) to market FW services and engage clients in
Completes Family Needs Assessments and determines level of service
Refers clients to external services or internal services based on service
Provide service coordination of internal and external referrals and
Assist clients with the development of identified goals and assisting them with the resolution of any identified barriers to goals. Works with clients to achieve outcomes in various service
Conducts the initial outreach for all lease compliance
Links clients with transit support and provides referrals to community service providers and E&ES partners when
Participates in staffings conducted by the Program
Other duties as assigned
Ideal Candidate Qualifications:
Have a Bachelor’s degree or higher from an accredited College/University OR related experience.
Have extensive outreach experience and proven high engagement with CHA
Bilingual (Spanish-English) required
Be self-motivated and be able to effectively market FW services to
Have excellent presentation, communication and computer
Have experience working with diverse, at risk, and vulnerable
Be knowledgeable of the community resources that offer self-sufficiency, youth empowerment services, and quality of life
Posses a valid driver's license, automobile insurance, and reliable
Work well independently and as a team
Have communication skills that lend themselves to building effective partnerships with CHA and related service
