Essential Duties & Responsibilities
Includes the following:
Complete comprehensive psychosocial assessments, identitify needs, provide referrals, and assist clients with application process.
Conduct in home assessment with DRS customers to meet customer need for assistance with activities of daily living eligibility for program services. Conduct in office assessment for Medical customers.
Provide monthly home visit for DRS customers to ensure customer health and safety, supportive services needs are met.
Provide education, information and linkages to programs.
Assess customer eligibility for financial assistance programs and assist customer with applications as appropriate.
Review/revise service plans and perform follow-up to determine the quality of services provided to customer.
Provide and support customer in the follow up with medical care, medication adherence, risk reduction, nutrition, mental health and substance use.
Locate, identify and utilize community services and program.
Develop and maintain required records, report and continuing education requirements. Update client information in client centered data base within 3 business days of completion

A bachelor degree in Sociology, Psychology, Social Work and/or equivalent experience or training in medical/nursing or related field preferred.
Previous experience in HIV prevention services and knowledge of working with various populations, LGBT, and HIV+/PWA required.
Driver’s License, insurance, good driving record and access to a reliable transportation.
The candidate must pass the Standardize Case Management Competency Training provided by the AIDS Foundation of Chicago within a year.
Must possess excellent interpersonal, organizational, communication and analytical skills and computer literacy required with proficiency with Microsoft Office products preferred.
