The Social Service Coordinator provides casework and case management services to residents living at Walnut Place and Frank B. Peers buildings in Highland Park.
This position is part-time, 4 days/week.
Work takes place in 2 different residences: Walnut Place and Frank B. Peers, including the Social Service Coordinator’s office, the community space, and in client residences. Apartments can vary in cleanliness. Noise levels are reasonable.
Provides individual and family counseling/case management to older adults and their families.
Completes psychosocial diagnostic evaluation of situations where client is at risk and intervention is necessary, provides emergency assistance and clarifies options for clients and families.
Coordinate with hospital staff regarding discharge planning for tenants returning to Walnut Place and Frank B. Peers.
Implements and monitors care plans, coordinating community resources with other supportive services.
Maintain social service files in accordance with HUD requirements and be familiar with polices regarding service provision.
Communicate regularly with Evergreen Real Estate staff.
Facilitate semi-monthly group meetings for tenants.
Attend all required HUD meetings and trainings.
Makes referral to Adult Protective Service agency when abuse, neglect or exploitation is suspected.
Completes statistical data and all required paperwork in a timely manner.
Participates in weekly supervision with Program Supervisor.
Attends scheduled staff meetings.
Completes required (per HUD guidelines) hours of continuing education each year.
Provides educational services to community groups and professional agencies.
Responds to medical emergencies and performs CPR/AED and First Aid if certified.
Performs other duties as may be assigned.
Bachelor’s Degree in Human Services or related field
Licensed Clinical Social Worker or Licensed Clinical Professional Counselor, preferred
Two years’ experience in social services provision to older adults, required
Sensitivity to cultural diversity
Knowledge of basic computer programs
Knowledge of supportive services and programs, such as Medicare and Medicaid
Current driver’s license and access to a car, required
Free of communicable diseases as determined by an annual TB test
Clears criminal background check
Capable of making visual assessments of the home environment
Ability to lift and carry 25 lbs.
Ability to climb stairs
Current CPR/AED and First Aid Certification, preferred
Ability to work independently

We didn't know what it would look like, but we knew that creativity, enthusiasm and knowledge would be its foundation.

Today, thanks to the collective efforts of our members, staff, volunteers and friends, the North Shore Senior Center community we imagined is a reality. Whether you visit us to take a class, enjoy a concert, or exercise in our Fitness Center, you can experience its benefits.
We also imagined that our community could extend beyond the walls of our brick-and-mortar buildings. It was important that older adults who were isolated due to physical, emotional or financial issues could participate as well. It was equally important that North Shore Senior Center could help those who needed assistance with Medicare, income taxes, caregiving concerns and issues relating to Alzheimer's disease and memory loss.
To ensure that all the pieces we imagined fit together perfectly, we strengthened our community with the passion of volunteers and the commitment of our partners in local businesses and government.
