POSITION SUMMARY: Provide progressive case management services to members of families who are homeless residing in the SRHAC – Interim Housing Program for families; facilitate intake, complete family assessment; develops individual service plans; assist families with accessing needed resources; provides crisis intervention; maintains case records; advocates and research additional resources to assist family with ending homelessness

Complete Family Assessment with 24 – 48 hours of Intake
Maintain weekly formal contact with each family
Maintain daily informal contact with families as needed
Linkage to mental health services and/or substances abuse services where appropriate
Linkage to employment assessment and job training programs
Public Benefits Screening and acquisition
Crisis and domestic Violence counseling referrals
Input documentation daily into the database
Other duties as assigned by supervisor when needed
Treat all members of the program with dignity and respect
Fulfill all responsibilities as a mandatory reporter
Display a positive attitude with members of the program
Demonstrate punctuality and good attendance
Possess good organizational, time management, and excellent interpersonal skills
Must be able to work under pressure and use good judgment at all times
Self-motivated with the ability to work without direct supervision
Physical ability to navigate stairs – including frequent visits to the 4th floor without elevator access
Must possess good organizational skills with developed problem solving skills and ability to prioritize multiple demands.
Must possess self-motivation and independent work habits.
Must exercise confidentiality regarding the affairs of families, staff, and SRHAC business.
Must possess strong leadership/facilitation skills.
Knowledge of de-escalation and conflict resolution methods.
Must be able to lift, stand, and bend when needed; and be to walk up the stairs to the 4th floor and back
The Single Room Housing Assistance Corporation (SRHAC), a City and HUD –funded 501(c) 3, was created in 1995 as a grassroots initiative by SRO operators/owners to bring together the for-profit, not-for-profit, and governmental sectors into a unique partnership to assist the SRO community in finding solutions to: (1) Help strengthen, preserve and protect Chicago’s SROs as vital, affordable housing...; (2) The help homeless and disabled needy individuals find SRO housing and connect with supportive services; (3) To help prevent single adults living in SROs from relapsing into homelessness by stabilizing and improving their lives; (4) To assist and advocate for SRO residents and SRO operators; (5) Top combat homelessness.
