Primary Function:Under the guidance of the Director of Social Services, provides overall administration and supervision of Care Coordination staff in all contracted areas.
Primary responsibility for overall program compliance for Community Care program, Care Coordination, VIP, Colbert Consent Decree, and other grant projects.
Responsible for the provision of quality assessments, case management activities and other clinical programs delivered under the State of Illinois Care Coordination and Age Options contracts.
Primary responsibility for the supervision of care coordination staff.
Primary responsibility for training of staff and students on CCP, Care Coordination; coordinate with other staff to insure that IDOA protocols are met for care coordination certifications. Scheduling of in-services and department meetings for social service staff.
Responsible for the integrity of all billing with the Illinois Department on Aging Care Coordination, Community Care program, VIP, Colbert Consent Decree, and other grants.
Provides Director of Social Service and fiscal staff information on billings submitted on a bi-monthly basis.
Oversite of data entry staff and compliance with standards and timeframes for submissions.
Maintain the confidentiality of care coordination records in compliance with ethical principles. Maintain written records for compliance reviews.
Provides monthly reports to the Director of Social Services on unit activity, quality improvement measures, CMIS management reports and other issues or concerns.
Responsible for development of new processes and procedures to meet changing demands and grant requirements.
Attends ICCCU meetings in Springfield
May act as a back-up supervisor for Adult Protective Services.
Maintains effective and timely communication with the Director of Social Services regarding inter and intra departmental issues.
Performs such other duties as may be required or assigned by the Director of Social Services.
Master’s degree from an accredited college or university in social work, counseling or related field.
Minimum 3 years experience in providing services to the older adult population.
Ability to communicate effectively with clients, families and other community service providers.
Successful completion of Adult Protective Services training and the State of Illinois comprehensive assessment training.
Completion of 18 hours of annual in-service training related to the field of aging.
Aging Care Connections is a community based organization in suburban Cook County. ACC offers a wide range of programs and services to older adults and their family members. Programs and services are funded through State of Illinois contracts, Federal grants and private grants.
