As a Community Support Team Lead, you will support people to live in and be a part of their community. You will receive a smart phone and laptop and access to a PACE vehicle for work travel. You will receive weekly clinical supervision required for licensure provided as part of employment, free on-site CEU's offered at least 4 times/year, competitive wages, 11 paid holidays, and generous paid time off.
Job Qualifications: * Master’s Degree in a Human Services related field. * Minimum of one year experience working with individuals with mental illness and working knowledge of the recovery model preferred. * Experience with Medicaid Rule 132 documentation and billing requirements preferred. * Current license in Counseling or Social Work preferred. * Valid Illinois driver's license and documentation of current auto insurance, with good driving record and car available.
Position Responsibilities: * Manage daily schedule of consumers (clients served) home visits. * Facilitate weekly team meetings. * Support staff in handling consumer crises. * Complete assigned program reports. * Conduct consumer home visits (monitor mental health, medication adherence, basic needs, money management, etc.). * Provide psychotherapy in natural environment. * Link consumers with additional resources (psychiatry, substance use treatment, food pantries, community centers, doctors, etc.). * Locate housing. * Monitor transition of consumers from long-term care settings to independent community living. * Complete authorizations for services with managed care companies. * Complete annual Mental Health Assessments for consumers on assigned caseload. * Collaborate and develop individualized Treatment Plan every six months for consumers on assigned caseload. * Respond to crisis calls in the evenings, weekends, or holidays as assigned. * Document all consumer services in electronic clinical record.
