Participates in initial home visit with assigned Parent Educator
Assists parents with understanding and implementing the Family Partnership Agreement in order to encourage and promote their overall development including achievement of self-sufficiency as well as positive developmental outcomes for their children.
Conducts periodic home visits and phone calls throughout the year to assess family resources and needs.
Identifies services currently available and those needed by families enrolled in the Early and/or Head Start program.
Develops relationships with other community-based service providers targeting parenting families.
Refers and links children and parents to needed services including but not limited to (EI/LEA, WIC, housing, food pantries, medical clinics, etc.)
Works in collaboration with other SGA staff and consultants to monitor, track and coordinate services for children and parents.
Responsible for data entry and monitoring of child health, immunization, dental, disabilities, and community referrals utilizing the Childplus system.
Informs families when required physicals, immunization records, etc. are due and conducts home visits to pick up physicals.
In collaboration with the Parent Educator, implements protocols to follow-up on absenteeism.
Participates in staffing and/or family child reviews of all families on their caseload and case studies.
Maintains records and documents services in a timely manner.
Serves as an advocate for Early and/or Head Start families and as a liaison between families, the program, and the large community.
Facilitates parent’s training and promote parents’ overall involvement and participation in the trainings.
Assists in planning and implementing parent activities during group socializations, and encourage parent participation in program activities and obtain feedback from parents regarding the level of satisfaction with the services they receive.
Participates in on-going training and staff development provided by the Ounce, Family Support Coordinator, EHS & HS Director, other consultants, workshops and conferences.
Attends regularly scheduled staff meetings, family support meetings, social service and health meetings.
Assists in identifying children with special needs and makes the necessary referrals.
Participates in the evaluation of Early and/or Head Start services, and ensures compliance with Early and/or Head Start Performance Standards.
Contributes to the Agency’s efforts related to prevention and integrated service delivery with other community providers. Illustrates knowledge of the Agency’s Continuous Quality Improvement Activities.
Supports and contributes to inter-system collaboration with other service providers to improve social service delivery systems.
Collaborates with Education staff to facilitate children’s transitions.
Collaborates with Disabilities Agencies to recruit children with disabilities into the program.
Collaborates with Health and Nutrition Consultant to ensure health and nutrition needs of children and families are addressed.
Completes other duties as assigned.
QUALIFICATIONS FOR THE JOB
AA, AAS, BA, or Advanced degree in Social Work, Family Services, Family Studies, Human Services, Social Services, Counseling or in related field; or expected completion within 18 months of hire.
Experience working with young children is required.
Sensitivity to cultural diversity is required. Ability to communicate and cooperate with diverse families, various professionals and community groups.
Must be able to make home visits to present and prospective and former clients in the clients’ own home.
Ability to write routine reports and correspondence.
Ability to speak effectively before groups of customers or employees or the organization.
Exercises discretion and independent judgment in executing tasks.
Must demonstrate the ability to establish helping relationships with families of varying educational, experiential and socio-economic backgrounds and handle emergency and crisis situations.
Competent in Microsoft Office tools and inputting services in an online database.
Schedule may be flexible and may include some evening and weekends.
Valid Driver’s License and Automobile Insurance.
Must be able to remain stationary 50% of the time.
Must be able to frequently move inside the office to access file cabinets, office machinery, etc.
Must be able to transport oneself to and from home visits.
Occasionally ascends/descends stairs.
Constantly positions oneself to assist children during activities.
Frequently moves desks, chairs, tables, easels, and children's toys.
At times may be required to lift up to 25 lbs. for various classrooms and events needs.
