Under the direction and supervision of the CTL and Manager of Patient Centered Care, the HIV/HCV Program Coordinator will be responsible for overseeing/coordinating the referral and scheduling process for liver elastography. This position will be responsible for performing the FibroScan procedure, completing all relevant paperwork; facilitating communication between the HIV primary care providers and HCV providers, to maintain continuity of patient care. He/she will provide HIV/HCV trainings and health promotion awareness. He/she will submit monthly reports of program activities to the funder and other administrative staff.
Principle Duties and Responsibilities:
Responsibility: Manage Internal and External Hepatitis Care and Management Referrals.
Schedule outside referral patients into Scheduler
Follow-up patient appointment non-compliance
Coordinate with CORE providers to schedule patients who completed treatment to receive FibroScan
Ensure that outside referral sources know what referral form to use, and how to submit.
Serve as liaison between providers, centers and hospitals.
Respond to incoming telephone calls regarding pending referral/results, including timely response to voice mails and other messages.
Complete patient empanelment for each weekly HCV clinic session for pre-clinic meetings to determine patient needs.
Responsibility: Manage the FibroScan Liver Elastography staying at CORE Center.
Perform FibroScan procedures for Hepatitis Clinic, internal and external referral sources, and Cermak Health.
Assist with training staff in conducting FibroScan procedure.
Ensure all FibroScans are interpreted, signed and scanned into Cerner.
Communicate accurate and timely results to providers
Confirm all information (including schedule appointment, registration with FIN#, FibroScan orders, interpretations and actual FibroScan with reference range) are documented for billing purposes.
Ensure yearly scheduled maintenance of FibroScan machine.
Provide linkage to care for FibroScan patients staged for fibrosis.
Attention to personal privacy and no aversion to skin contact when preparing patients for procedure.
Responsibility: Provides patient and staff education around HIV, HCV and management of the diseases.
Ordering of educational materials and promotional materials for patients and staff.
Promotes and explains HIV/HCV projects and objectives to medical providers and other members of the multi-disciplinary care team and to external community partners.
Responsibility: Manager Hepatitis Database and other data requests.
Ensure patient information is entered during patient procedure and uploaded into Power BI at end of the session.
Provide data report to funders to HCV regarding: 1) total FibroScans for previous month, 2) Number of scans attempted & reason for non-completion, 3) Number of scans completed, 4) Breakout on Fibrosis stages for both internal and external referrals.
Other Duties as Assigned
Licensed Practical Nurse or Medical Assistant background or a minimum of two years of experience in health care settings, health education, HIV counseling and testing is required. Proficient in use of Microsoft Word, MS Office (Excel, Outlook) and PowerPoint is required. Experience with ACCESS database and Power BI is preferred. Experience working with diverse population, within a multicultural and multidisciplinary team. Must have strong communication (oral and written), organizational, interpersonal and problem solving skills with good attention to detail. Knowledge of HIV/HCV medication, treatment, and insurance limitations for treatment. Experience as a technician in staging of liver disease with FibroScan technology is required.
