The Manage of the Maternal Child Health (MCH) Program oversees care management programs to high risk pregnant and parenting women, infant and children to affect the delivery of health and supportive services in an integrated manner.
The MCH Manager serves in an expanded health care role to collaborate with members of the health care team, and patients/families to ensure the delivery of quality, efficient, and cost-effective health care services.
The position integrates evidence-based clinical guidelines, preventive guidelines, and protocols, in the development of individualized care plans that are patient-centric, promoting quality and efficiency in the delivery of health care.
Will follow role assignments that are in alignment with the Patient Centered Medical Home (PCMH) Standards, and participate in all Patient Centered Medical Home efforts.
Oversees the assessments of patients, development of care plans. Implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patients’ health status.Oversees staff conducting monthly and/or as needed Home Visits to each participant
This position manages a caseload of approximately 60 high-risk pregnant women and their infants, and children up to age 2 year, per primary care provider (PCP) referral, risk stratification, and/or patient lists. May directly provide medical care to patients if appropriate NNHSC Board Approved Privileging and Credentialing are complete
Provides targeted interventions to avoid hospitalization and emergency room visits. Coordinates care across settings and helps patient/families understand health care options.
Identifies the targets high risk population within practice site(s) per PCP referral, risk stratification and patient lists. Includes patients with repeated social and/or health crises
Coordinates patient care through ongoing collaboration with PCP, patient/family, community, and other members of the health care team. Fosters a team approach and includes patient/family as active members of the team
Provides follow-up with patient/family when patient transitions from one setting to another. Completes timely post-hospital follow-up: Medication reconciliation, PCP or specialist follow-up appointment, assess symptoms, teach warning signs, review discharge instructions, coordination of care, and problem solve barriers
Bachelor of Science in Nursing (BSN) or Master of Social Work (MSW) degree.
LCSW or RN (Illinois license)
Saturday hours, evening hours and/or may be required to work at a different site
