The HIV Testing Counselor will identify and target high-risk populations for HIV testing. Conducts education workshops in the community to promote awareness of HIV \AIDS.
Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards, and participate in all Patient Centered Medical Home efforts.
Conduct pre-test and post-test HIV/AIDS counseling to participants. Ensure clients return for post-testing counseling and results.
Help participants identify their risk for acquiring or transmitting HIV and other STI’s.
Prepare participants to receive, understand, and manage their test results.
Maintain accurate reports and records regarding outreach activities, test results obtained, and follow-up activities.
Participates in community events and meetings.
Coordinates and/ or facilitates regular education and informational workshops.
Other related duties.
AA degree preferred, or at least two years of college
Bi-lingual (Spanish) a plus
Completion of an accredited course in HIV/AIDS counseling prevention, and/or education and certification in the field of AIDS/HIV infection counseling.
Current driver’s license, and able to provide own transportation.
