Provides intensive Case Management/BBO (Better Birthing Outcomes) services to high risk pregnant and their newborns to affect the delivery of health and supportive services in an integrated manner. Will work closely with the Case Management Coordinator to ensure programmatic requirements for home visits are met.
Works with high risk pregnant women and their newborns to assure a healthy delivery and postpartum care by providing intensive home visitation and case management services.
Maintains an assigned caseload.
Completes individual assessments to determine the level of the participant’s health & service needs.
Links participants to needed services; monitors service delivery and participant’s compliance.
Documents participant’s encounters and data (Includes electronic health record and cornerstone systems documentation).
Participates in community workshops, conferences, seminars, school programs and health fairs.
MSW, MPH, LCP, LCPC, or RN required.
