Provide education, outreach, prevention, and case management to improve health and wellbeing of immigrants, refugees, and other underserved populations. Play an integral role in ensuring that the immigrant and refugee communities maintain quality of life.
Basic Job Function:
The Housing Advocate will assist in the administration of program activities in the HIV Care - Housing Resource Program. Tasks and duties will be assigned by the HIV Care Program Manager in collaboration with CH Director. The Housing Advocate will be working with the Housing Resource team, Community Health program staff, and other AHS staff to ensure the full circle of services to the clients and the community.
Duties and Responsibilities:
Assist HIV Care Program Manager in updating and developing program tools;
Implement project scopes of services and ensure compliance;
Produce and submit monthly, quarterly, and annual reports, as required, in a timely manner;
Maintain and update client databases, files, and required documentation;
Maintain and update the housing directory and resources;
Contact service providers and property managers to find and update community and housing resources for client linkages;
Conduct intake and assessments and develop working service plans for new and existing clients;
Provide confidential and culturally sensitive services in working with clients and the information gathered for client files and housing applications;
Assist in the development, organization, and implementation of workshops for clients;
Attend required team meetings and program planning meetings;
Meet program related training requirements;
Adhere to agency missions and policies;
Other duties assigned as required.
Bachelor’s degree or equivalent experience in social work;
Health education, and related fields with at least 2 years of experience;
Bilingual and/or experience working with HIV population or housing placement is preferred.
Physical Requirements and Working Environment:
Able to drive, with valid Illinois driver’s license and body/liability insurance; or use public transportation when needed.
HIV services and policies;
Updated with training on HIV care and treatment.
Good client/customer service skills;
Being able to multi-task and complete tasks in a timely manner;
Ability to work in a team, as well as independently;
Excellent communication skills.
