Continuity of Care is a planned and carefully synchronized process vital to high quality care which involves the participant, family, and an interdisciplinary team.
The Continuity of Care Coordinator has primary responsibility for the continuity of care process and coordination of services for participants. The coordinator will assist with organizing and leading interdisciplinary consultations and providing an action plan that outlines the agreed upon recommendations. The coordinator will review and authorize comprehensive mental health assessments that demonstrate medical necessity and determine service needs. The coordinator will also review and authorize treatment plans to ensure they include the identified goals and service needs of the participant and their family. The Continuity of Care Coordinator will make clinical recommendations to staff regarding the MHA’s and ITP’s to promote the highest quality of evidence informed practice. The Coordinator also functions as the licensed lead for the Williams Quinn Clinical Review Team.
Master’s degree from an accredited college or university in social work, psychology, human services or a related field.
Three years of experience in the mental health and/or substance abuse field.
Experience working with persons with severe mental illness and substance abuse.
Prior experience in a multi-ethnic setting preferred.
Daily use of a car preferred.
Flexibility to work on weekends, evenings, holidays during emergencies.
