Providing co-located and collaborative behavioral health services to patients at the Pillars Community Health Medical Center. Provides outpatient mental health assessments and group treatment services to a caseload of clients.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Completes comprehensive mental health assessments and coordinates group treatment for clients within the Behavioral Health program.
Responsibilities include: providing mental health care, including comprehensive assessment, treatment planning, case management services and group therapy.
Facilitates behavioral health groups, including tracking paperwork deadlines for group-only clients.
Collaborates with referring medical providers and social workers on site to provide continuity of care and collaborative treatment for clients, including providing information about referrals, treatment progress, diagnosis, and treatment completion when necessary.
Provides curb-side consultation to medical providers regarding mental health needs of medical patients.
Obtains proper releases of information and ensures HIPAA compliance and privacy compliance.
As needed, provides individual and family therapy, to group clients in the integrated care program.
Completes mental health assessments in order to formulate an accurate diagnosis and comprehensive treatment interventions to serve the identified client and family.
Determines frequency, duration and intensity of treatment, and develops treatment plans.
Assesses and stabilizes high risk individuals and/or families.
Works in collaboration with other agency programs and staff.
Coordinates with other community agencies and organizations as needed such as but not limited to DCFS, schools, hospitals, and other providers in the community.
Provides case management services as defined in the service plan that coordinate, support, and advocate for clients who need multiple services and require assistance in gaining access to and in using mental health, social, vocational, educational, child welfare, and other community services and resources.
Responds to clients and/or family needs outside of regularly scheduled session times.
Monitors and supports services provided by psychiatrist.
Maintains records in accordance with agency policies and complete required outcomes protocols while meeting established productivity standards.
Participates in agency meetings and in-services.
Other duties as assigned.

EDUCATION AND EXPERIENCE
Master’s degree in Counseling, Psychology, Clinical Social Work, Marriage and Family Therapy or related degree required. Experience delivering care in a medical setting preferred. Experience working with mentally ill children, adolescents and adults required. Training in and experience with a variety of evidence based treatment models required. Ability to work as part of a team and collaborate with consumers, referral sources, and other community providers is essential.
Bilingual (English/Spanish) preferred but not required
