The Case Manager provides case management services to high risk pregnant women and their newborns to affect the delivery of health and supportive services in an integrated manner. The Case Manager will work closely with the Maternal Child Health Manager to ensure programmatic requirements for home visits are met.
Works with high risk pregnant women and their newborns to assure a healthy delivery and postpartum care by providing intensive home visitation and case management services.
Maintains assigned caseload.
Completes individual assessments to determine the level of the participant’s health & service needs.
Service planning (i.e. development of care plans) and resource identification as applicable.
Links participants to needed services; monitors service delivery and participant’s compliance.
Documents participant’s encounters and data (Includes electronic health record and cornerstone systems documentation).
Participates in community workshops, conferences, seminars, school programs and health fairs, staff development activities, and case conferences, as appropriate.
Bachelor's degree and reliable transportation required. Experience in community health or maternal and child health. Bi-lingual (English/Spanish) preferred.
