Position Summary: Illinois Breast and Cervical Cancer & Wise Woman Programs staff will help uninsured and underinsured women access free mammograms, pap-tests, and preventative health services for cholesterol, diabetes, and other cardiovascular diseases at Asian Human Services Family Health Center, Inc. Primarily, the staff will be responsible for providing one-on-one assistance to patients before and after doctor appointments to ensure that they receive necessary support and resources. IBCCP/IWP staff will also work in collaboration with local hospitals, community centers, contracted providers, and AHS Family Health Center staff to provide services offered through IBCCP and IWP. Additionally, the staff will support IBCCP/IWP Coordinator by assisting with client intake, data- entry, health-education, and outreach efforts to promote IBCCP & IWP programs. The staff’s service will be directly at supporting at-risk women, many of whom are immigrants and non-native English speakers. Overall, staff will be providing patients with quality support, resources, and information on breast and cervical cancers, as well as overall health and wellness.
Complete client assessments, take demographic information, and document relevant medical histories for new/established IBCCP/IWP patients before/after doctors’ appointments
Explain IBCCP/IWP services and ensure that each patient is referred to appropriate service providers depending on their individual medical need or care-plan, including referrals from internal and external agencies or other departments
Assist patients in scheduling additional appointments at local clinics and hospitals contracted with IBCCP/IWP to provide mammograms, pap-tests and preventative health services for diabetes, cholesterol, and other cardiovascular diseases (CVD)
Provide ongoing follow-up with patients and/or service providers to determine whether patients have appropriately accessed services and met their goals.
Contact patients to ensure that they understand the results and schedule follow-up appointments as needed
Provide comprehensive support and resources to clients from initial identification to case closure
Update patient information in the Cornerstone data-entry system in a timely fashion
Collaborate with various service providers and utilize clinic/community resources to assure comprehensive and coordinated care
Assist in answering the phone, making copies, and scanning IBCCP/IWP progress notes, lab results, and X-ray results appropriately
Participate in IBCCP/IWP/AHS Family Health Center meetings, workshops, and trainings
Assist in outreach efforts, including attending Health Fair representing the IBCCP/IWP program as well as Asian Human Services Family Health Center, Inc.
Excellent teaching and communication skills
Requires experience and demonstrated abilities for working in a multi-cultural setting.
Should have a strong commitment to the mission of Asian Human Services Family Health Center, Inc.
Ability to organize and prioritize work appropriately
Knowledge in social work, social/health sciences, or nursing is preferred
Knowledge of second language preferred
Past experience of working in a medical office is required
