AgeOptions seeks four dynamic individuals to work as Health Education Facilitators within Cook County and surrounding suburbs. Under the leadership of the Health Education Program Manager and directly reporting to the Health Education Implementation Coordinator, the Health Education Facilitator is responsible for conducting outreach to recruit workshop host sites, facilitators and participants, and for facilitating Take Charge of Your Health /Diabetes workshops. Health Education Facilitators provide information and tools so that people can take steps to improve their own health.
This position will not have set work days or hours (TBD), but will generally average 10-15 hours per week. This position is funded with grant funds which currently last through at least July 1, 2020.
Facilitate Take Charge workshops primarily throughout Cook County and parts of DuPage County, at churches, senior centers and other locations
Coordinate with co-facilitators to recruit and retain participants and successfully deliver workshops
Assist with distribution of marketing materials, meet with staff at local community workshop host sites to assist with recruitment and retention of workshop participants
Represent AgeOptions at outreach meetings as requested
Other duties, as assigned
Bachelors Degree in a related field
3-5 years experience as educators/facilitators
Bi-lingual in Spanish preferred
Must be able to have flexible schedule to accommodate workshops on various days of the week. Most workshops are on weekdays.
Ability to successfully become a certified Take Charge of Your Health and Take Charge of Your Diabetes facilitator trainings and/or Tomando Control de su Salud and Tomando Control de su Diabetes, as appropriate. Training provided.
Ability to travel within Cook County, IL, and outlying collar counties as needed. Must have reliable transportation, valid driver’s license and proof of insurance
Ability to deal effectively with workshop participants
Ability to work cooperatively with community based organizations
Ability to effectively communicate
Strong training and organizational skills
