The AIDS Foundation of Chicago (AFC) mobilizes communities to create equity and justice for people living with and vulnerable to HIV and related chronic diseases. We envision a world in which people living with HIV and related chronic diseases will thrive, and new HIV infections will be rare. Our work focuses on improving health equity, preventing new cases of HIV, serving as a collaboration and knowledge center, and being a bold voice for change.
We are seeking for an individual to serve as our Certification Specialist. The Certification Specialist is responsible for certifying clients for Ryan White Part B service eligibility every 6 months, at the AIDS Foundation of Chicago as well as at subcontractor agencies based in Cook County as well as the Collar Counties. The first and highest priority of the medical case management system is to ensure that persons living with HIV/AIDS enroll in and sustain coordinated health care in order to optimize their health and well-being. The Certification Specialist will also assist the Ryan White Program Manager in managing client databases ensuring eligible clients are linked and retained in care.
Principal functions and responsibilities of this position include the following:
Assessments & Service Plans
Conduct face-to-face and over the phone assessment and reassessments every six months;
Create service plans that match the identified needs of the client;
Ensure that all clients referred to services are eligible prior to the date of service.
Refer clients to appropriate services highlighted in the service plan;
Refer clients to more intensive case management services if needed;
Refer clients seeking services through the client database to the appropriate service;
Complete emergency housing applications for eligible clients;
Document encounters using designated client database;
Collaborate with agencies on setting times and dates at their agencies to meet with clients;
Schedule client appointments both at AFC and subcontractor agencies;
Research client’s insurance and verify that designated service is not included within their plan;
Compile a list of new agencies that serve persons living with HIV/AIDS and conduct outreach;
Travel to all funded agencies;
Provide technical assistance to case managers in regards to the client database;
Complete at least 12 hours designated on-going training annually.
Assist with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, and others;
Protect organization's value and manage risk by keeping information confidential;
Perform other duties as assigned.
Associate’s degree in a human services field OR at least one year of experience in Social Services, Health Care or related field.
Basic knowledge of HIV/AIDS prevention and treatment.
Bachelor’s degree in the Human Services field (i.e., Psychology, Sociology, Public Health, Social Work).
Knowledge, Skills, and Abilities
Strong attention to detail and the ability to manage multiple projects, establish workload priorities, and meet deadlines are critical.
Excellent customer service skills and strong interpersonal skills and the ability to develop and maintain professional, trusting and positive working relationships with both internal and external customers.
Knowledge of Microsoft Excel and/or other database programs. This includes the ability to generate basic reports using these programs.
Strong problem solving skills including the ability to define, diagnose, and resolve problems.
Also, strong verbal and written communication skills and ability to work with diverse populations are key to success in this role.

Anyone can become infected w...ith HIV, but HIV and related chronic diseases disproportionately impact certain populations in distinct geographic areas. We can have the greatest impact on the HIV epidemic by focusing our efforts on those most-impacted by HIV, based on epidemiological data and unmet need:
• Young Black gay and bisexual men
• Transgender women of color
• Black women living in high-incidence areas
• Latino gay and bisexual men
Factors that drive the HIV epidemic and health inequities that we will address include:
