About the role:
Reporting to the Health Education Integration Coordinator, the Health Education Integration Specialist assists the team with managing statewide programs including creating marketing materials in Spanish, cofacilitating workshops, maintaining relationships with our partners, identifying and developing workshop implementation tools and increasing the visibility of AgeOptions in the communities we serve.
This is a highly collaborative position that works in coordination with the Health Education Team. Key members of the team include the Manager of Community Programs who leads the team; the Health Education Systems Integration Coordinator who is focused on developing relationships for new partners to deliver the programs, IL Pathways to health website management and oversight of all TCYH data; the Health Education Implementation Coordinator who is focused on site implementation of workshops, recruitment of participants and executing all health care partner site workshops from registration to completion; the Health Education Program Specialist who monitors workshops from registration until completion, facilitator certification and availability for statewide partners and workshop providers and identifies volunteer sites and facilitators. This position will work closely with all team members so that workshops are successful, and funders and referral sources continue to support the program.
The primary health promotion program used by AgeOptions is the Take Charge of Your Health/Diabetes, otherwise known as CDSMP/DSMP. There are also culturally adapted Spanish versions of the programs (Tomando Control de su Salud/Diabetes).
Assists with facilitation of local outreach and engagement activities.
Assists with development and implementation of marketing materials use to recruit host sites, participants and facilitators.
Facilitates Tomando Control de su Salud and Tomando Control de su Diabetes workshops.
Works with other AgeOptions team members to facilitate a coordinated outreach to Latinos.
Assists with maintaining and developing strong collaborative relationships with partners that contribute to the mission of building more capacity for Tomando workshops in Cook and collar counties.
Assist with outlines and input for grant applications.
Monitors branding and messaging for all delivery partners. Manages social media pages and trains others on media releases and social media.
Monitors workshop registrations and updates schedules on the Responsible for data entry of workshop data and evaluations.
Who you are
Bachelor’s degree and two years’ experience in communications, social services, health care administration, volunteer coordination or a related area.
Bilingual in Spanish and English; effective written and verbal communication skills.
Ability to deal cooperatively and effectively with multiple partnerships and community-based organizations.
Strong training and organizational skills
Knowledge of Workshop Wizard, Wufoo a plus.
Strong social media outreach skills (Facebook, Twitter and blogs).
Strong background in account management for reimbursement processing
Background in HIPPA compliance
Ability to travel throughout Illinois. Must have reliable transportation, valid driver’s license and proof of insurance.
