Actively contribute as a clinician on a team providing intensive, recovery oriented community based behavioral health services to persons living with serious mental illness. Effectively engages consumers in treatment planning that fosters hopefulness and motivates change. Provides group and individual skills training that targets areas of functional impairment and promotes independence through the development and use of natural community supports. Specific team assignments will be determined based on clinician expertise, experience, interest and credentials. Provide clinical and administrative oversight to a team of part time behavioral support specialists as they provide skill building services to adult clients in the community. Coordinate with human resources to maintain high quality staff, including marketing and hiring of new staff. Provide program orientation, monitor staff documentation, complete payroll and lead team meetings for group supervision. Partner with clinicians and supervisees to translate treatment plans into behavioral interventions. Provide, as needed, individual supervision and complete timely performance evaluations for supervisees.
Lead/co-lead on-site groups within the psychosocial rehabilitation program (PSR) targeting acquisition of skills that support and/or sustain client recovery
Provide care coordination and a full range of direct services including referrals of additional services and consult with other providers for a caseload of adult clients living with serious mental illnesses (primary clients)
Complete or participate in completing face-to-face mental health re-assessment as needed but at least annually for primary clients
Complete or participate in completing timely treatment planning for primary clients as needed but at least every 6 months.
Actively participate in team consultation, operations and planning meetings
Promote family, partner or significant other participation in care whenever possible and clinically appropriate
Work cooperatively with other Center units/staff to maintain smooth continuity of care when cases are shared
Accept and process referrals, Work with clinicians to determine appropriate community based interventions and the best mentor match for each client. Provide clinical orientation for behavioral support specialists to cases when they are assigned. Orient clients to the program in treatment planning sessions.
Consult with other clinicians in team meetings and individually to monitor cases and advocate for behavioral support specialists.
Receive input from supervisor and provide feedback regarding team issues. Participate in annual performance appraisal.
Coordinate with HR, Information Systems and other agency departments to ensure adequate staffing, training and operations of the Adult Behavioral Support Team. Provide initial program orientation to the new supervisees.
Coordinate day-to-day operations and activities of supervisees.
Provide performance evaluations for supervisees.
Lead monthly team meetings for behavioral support specialists.
Participate in Adult Recovery Team meetings in order to generate referrals and provide feedback on mentoring services.
Individual Performance Requirements
Meet with Supervisor for clinical and administrative supervision, to receive and offer feedback and for performance evaluation
Meet or exceed productivity target.
Meet agency standards for timely billing by completing all direct service documentation within the week the service is provided.
Complete required assessments, treatment plans, and other documentation in an accurate, compliant and timely manner
Participate in orientations, all staff trainings, and training and development seminars
Interpret and communicate, at the service delivery level, Center policies and procedures
Submit, in a timely and accurate fashion, schedules timesheets, mileage and other reports required by the Center’s policies and procedures
Perform assessments of adults with serious mental illness
Completion of comprehensive behavioral health re-assessments and treatment plans
Ability to successfully work as a member of a multidisciplinary team
Ability to provide an array of clinical and concrete services, coordination, planning and transition and discharge planning
Working knowledge of community resources and services
Demonstrate the ability to consistently meet compliance requirements.
Program planning, development, and evaluation
Demonstrated knowledge of adult mental illness and treatment modalities including Skills groups, Illness education, recovery stages of change and WRAP (wellness recovery action planning)
Ability to establish rapport, systematically gather data, determine the client’s readiness for treatment and change,
Ability to design/plan training curricula and lead/co-lead skills and sustaining care groups that meet the needs of our targeted population.
Master’s Degree from an accredited college or university in social work, psychology, or counseling. LCPC or LCSW preferred or eligible within a year of hire.
Prefer 1-2 years supervisory experience.
Basic computer proficiency, knowledge of Word and internet skills
Exempt, Full-Time, 37.5 hour work week
Must work a flexible schedule in response to the needs of the target population served, including evening and Saturday hours.
