BEDS Plus Care provides emergency overnight shelter and support services including case management for homeless persons in southwest suburban Cook County communities. The Shelter Case Manager will oversee all emergency overnight shelter operations for BEDS Plus Care and serve as the primary point of contact for clients in the program. The Shelter Case Manager will work closely with Manager of Emergency Services & Outreach to provide case management services to shelter and newly housed clients.
· Shelter Preparation & Management
Serve as staff on duty at the emergency overnight shelter
Work with overnight shelter site coordinators to ensure that staffing at all sites is adequate, including site volunteers, cooks, and laundry personnel for each night
Oversee and ensure the care and proper treatment of the shelter site locations
Ensure all shelter paperwork is returned in a timely basis and submitted to appropriate staff
Appropriately address shelter and client matters in a prompt and professional manner
Client Case Management
Responsible for integrity of client files and data entry in HMIS
Ensure client intake process and secondary assessments are consistently and appropriated administered at emergency overnight shelters.
Create and document individualized client goal plans for BEDS clients
Connect BEDS clients with government benefits, medical providers, mental health services and housing options
Maintain a cooperative relationship with service providers
Responsible for reporting client data and outcomes on a regular basis
Part-time position – two evening & two daytime shifts per week.
Reports to the Manager of Emergency Services & Outreach
Primarily located at St. Mark Daytime Support Center in Worth.
Bachelor’s degree in social services field or equivalent prior experience with homeless population
