Active Visions, Inc. is a social service agency providing support to individuals with developmental disabilities and intellectual disabilities in home and community-based residential settings throughout Chicago and the surrounding suburbs.
The QIDP is responsible for providing case management services to individuals residing in their own home. As a QIDP, you are responsible for assisting in the delivery of services across settings in order to enhance the health, well-being, quality of life, opportunities and achievement of personal outcomes for individuals served. The QIDP is expected to uphold the Active Visions philosophy of individual choice and realization of person-directed goals.
The Home-Based responsibilities include but are not limited to the following:
Coordinate and lead home/day program visits with clients
Assist with the obtainment and maintenance of public benefits (SSI, Medicaid, foods stamps, etc.)
Develop implementation plan and oversee execution of the plan
Supervise Direct Support Staff working with individuals
Ensure proper documentation and upkeep of client files in audit-ready condition
Facilitate all services available in waiver program
Serve as a liaison between residents, staff, family and all other entities
Ability to communicate effectively, develop and maintain positive relationships with staff, clients, and other entities
Bachelor's degree in human services or related field (education, psychology, etc.)
Minimum one year experience working with individuals with intellectual/developmental disabilities
Access to a reliable car with insurance
