The Project Coordinator is responsible for coordination of the data-to-care project; assisting with the collection and processing of patient information for CCHIP (consolidated programs). This includes serving as liaison to the Chicago Department of Public Health HIV Surveillance staff, generation of appropriate reports to be shared with the Chicago Department of Public Health HIV Surveillance Section, and reviewing and analyzing data in consultation with appropriate Chicago Department of Public Health staff, CORE System Data Analyst, and Quality Management Committee. The position will coordinate patient information with EIS and outreach staff, and other linkage and retention in care programs, to assist in identifying patients that appear to be out of care. The position will also cooperate with the external evaluators to evaluate the Data to Care program and assist the System Data Analyst with CareWare and PROVIDE database reports.
Generates monthly lists of patients that are out of care and new patients that have not returned to care, in consultation with System Data Analyst.
Shares lists with Chicago Department of Public Health HIV Surveillance staff.
Attend regular meetings as scheduled by the CDPH to review the status of patients on the lists.
Shares patient information with appropriate CORE Center based linkage and retention programs to assist in the maintenance of current records regarding the status of lost to care patients.
Coordinates with the CORE Center’s various linkage and retention programs to collect information about patients identified by the Chicago Department of Public Health as lost to care.
Maintains liaison with and shares data with the program’s external evaluators.
Submits reports as required by the Chicago Department of Public Health and assigned by the System Data Analyst.
Maintains liaison with the Chicago Department of Public Health on all matters related to this project and communicate project status with System Data Analyst.
Participate in CORE Center internal meetings as required, inclusive of CQM Committee meetings.
Assist with maintaining the CORE Center’s CAREware database to ensure that data is of high quality
Participate in weekly QM/Data Team Huddles and monthly QM/Data Team meetings
Other duties as assigned that are reasonably consistent with the above essential functions.
Bachelor’s degree from an accredited College or University. Minimum of one years’ experience managing patient related data, including demographic, financial and laboratory data. Proficiency in Microsoft word, Microsoft Access/Excel required. Course work in epidemiology preferred. Familiarity with electronic sharing of data desired. Familiarity with HIPAA requirements desired. Experience in preparing client level and aggregate data reports for external funders or stakeholders preferred. Good oral and written communication skills. Self-motivated and proven ability to work with minimal supervision. Experience working with a culturally diverse work force including a variety of professional and levels of education.
