JOB SUMMARY: Under the direction of the Clinical Transition Liaison and the Manager of Patient Centered Care, the HIV/HCV Care Coordinator will work with the HIV and Hepatitis Clinic teams and outreach staff at CORE. This position is responsible for overseeing/coordinating the referral and scheduling process for liver elastography, performing FibroScans and completing all relevant paperwork. He/she will facilitate communication between the HIV primary care and HCV providers to maintain continuity of patient care. The HIV/HCV Care Manager will provide reminder calls to patients regarding appointments, labs, SVR, and medication adherence issues. He/she will work with the care team and provide patient education on medication, treatment, insurance limitations and keep the treatment database up-to –date. He/she will work closely with the CTL and Hepatitis team to provide staging (FibroScan) and linkage to HIV/HCV care and follow-up. He/she will ensure patient information is entered during patient procedure and uploaded into Power BI at end of clinic session. He/she will submit monthly reports of program activities to the Clinical Transition Liaison, funder and other administrative staff.
Coordinate with clinical staff from all CCHHS sites and other health care clinics to ensure linkage to HIV/HCV screening and treatment.
Calls patients to schedule for HIV/HCV screening and treatment.
Works with hepatitis clinic pharm D to call patients on treatment and submits paperwork for HCV drug assistance program
Follows up on patients lost to hepatitis staging and treatment.
Performs FibroScan procedures for Hepatitis Clinic.
Database entry and manage databases.
Ensures that all FibroScans are interpreted, signed and scanned into Cerner.
Provide patient, provider and community education on HIV and HCV related topics.
Serves as a liaison between providers, centers and hospitals.
Assists in preparation and presentation of services at local and national meetings.
Conducts outreach/home visits as needed to link patients to HIV/Hepatitis follow up care as needed.
Assists with connecting patient to a benefits counselor and reminds them to bring necessary documentation.
QUALIFICATIONS - Knowledge/Skills/Experience
Minimal Formal Education Required Minimum of a high school diploma or equivalent.
Minimal Length & Nature of Experience Required:Minimum of 5 years’ experience in health care settings or health education required. MA, LPN, Associate or Bachelor’s degree in health care field preferred. Proficient in use of MS Office is required. Experience working with a diverse population, within a multicultural and multidisciplinary team required. Must have strong communication (oral and written), organizational, interpersonal and problem solving skills.
Special Knowledge and Skills Preferred: Knowledge of HIV/HCV medication, treatment, and insurance limitations for treatment preferred. Experience as a technician in staging of liver disease with FibroScan
